YOU CAN REFER YOURSELF, OR SOMEONE YOU ARE CONCERNED ABOUT, DIRECTLY TO US…
You can refer yourself, or someone you are concerned about, directly to us. If the individual is an adult, we will need to contact them directly in order to schedule their initial appointment. Parents may refer children, under the age of 18, directly to us for services. Children may also be referred by their School Principal, Counselor, School Nurse or other concerned staff. Parents should be made aware of their child’s referral to our services. Individuals may also be referred by their Family Doctor, Judge, Probation Officer, Case Manager, Employer or from any agency concerned with accessing care.
REFERRAL INFORMATION…
Your referral information may be mailed or faxed directly to our office, or you may call and speak with one of our Customer Support Specialist who will help coordinate your referral by phone. After your referral information is recieved by our staff, an initial appointment for an Intake and Assessment session will be coordinated by a Customer Support Representatives. An initial appointment will be offered within seven (7) days of receiving your referral. A Clinician or Clinical Liasion will discuss treatments options with the individual completing the initial assessment session. A parent or legal guardian is required to attend this session with their child.
IF YOU OR THE PERSON BEING REFERRED TO A SERVICE IS AN ENROLLED CONSUMER WITH ANOTHER LOCAL CENPATICO BEHAVIORAL HEALTH PROVIDER…
If the child, adult or family is currently an enrolled consumer receiving services from another local Cenpatico Behavioral Health Provider. The Network provider agency is required to provide the following information with their referral:
- Current Assessment
- Annual Update
- Current Individualized Service Plan (ISP) identifying the service being requested
- Consumer’s Current Demographics
- Consumer’s Current Crisis Plan
- Strengths, Needs, Cultural Discovery

Su información se puede enviar o enviar por telefax directamente a nuestra oficina, o usted puede llamar y hablar con uno de nuestro especialista a coordenada su remisión por el teléfono. Después de que su información de la remisión sea recibida por nuestro personal, una cita inicial será coordinada. Una cita inicial será ofrecida dentro de siete (7) días de recibir su remisión. Requieren a un padre o a un guarda legal atender a esta sesión con su niño. |
